Summary/Abstract: The text examines informal payments as a key corruption practice in the Bulgarian healthcare system, characterized by widespread prevalence and ambiguous public perception – ranging between a bribe and an expression of gratitude. The analysis focuses on the forms, motives, and participants in these practices, as well as their occurrence at various levels of the healthcare system. Empirical data are presented, revealing a discrepancy between declared disapproval and the actual behavior of citizens. The report outlines major challenges related to the understanding and the restriction of informal payments
