Summary/Abstract: Post-traumatic stress disorder (PTSD) severely restricts an individual's functioning and remains one of the areas where modern psychotherapy struggles most to find solutions. This systematic review aims to examine the clinical effectiveness of Cognitive Behavioral Therapy (CBT), and especially its trauma-focused variations, in the treatment of PTSD from a quantitative perspective, based on current, randomized controlled trials. The studies analyzed show that CBT exhibits high success rates not only in traditional faceto-face sessions but also in internet-based guided interventions. Internet-based CBT (iCBT) was found to perform non-inferiorly to standard face-to-face therapy and resulted in significant reductions in CAPS-5 scores. Furthermore, the success of CBT in different cultural contexts (as in the case of Japan) and in socioeconomically disadvantaged groups (orphan children in Zambia) demonstrates the universal applicability of the intervention with quantitative data. Research findings reveal that CBT not only reduces core PTSD symptoms but also has a strong therapeutic effect on comorbid conditions such as sleep disorders, post-traumatic headaches, and HIV-risky behaviors. Analyses have shown that protocols supported by additional modules such as memory specificity training (MEST) or CBT for insomnia (CBT-I) reduce symptom severity by up to 40% in treatment resistant cases. Studies, particularly on veterans and primary care teams, numerically confirm that intensive CBT formats reduce treatment dropout rates and accelerate clinical improvement. This study scientifically demonstrates that CBT has a high adaptability to increasingly digital and complex clinical needs; and that with the right augmentation strategies and technological integration, it maintains its status as the "gold standard" in PTSD management.
