Summary/Abstract: Hyperacusis is a rare but disabling sound-tolerance disorder that disproportionately affects musicians exposed to high sound pressure levels. This paper outlines its classification and summarizes current knowledge on underlying mechanisms such as central gain enhancement and cochlear injury. A personal case of hyperacusis acquired during orchestral performance illustrates clinical presentation and challenges for professional musicians. Preventive strategies, especially the systematic use of hearing protection, are discussed alongside therapeutic approaches including graded sound therapy
