Individual care plans in gerontological social work

Summary/Abstract: This article reviews basic approaches to individual plan design for social work with older people. Objectives and areas of planning are discussed in the perspective of understanding of successful aging, quality of life and subjective well-being. A complex model of an individual plan is proposed that meets corresponds the social workers needs in the practice. The plan is structured in a usable form and implies a balance between care, support and own activity and autonomy of the client.
