Summary/Abstract: The aim of the present study was to explore and analyze the relationships between mental adjustment, Big Five personality traits, and Type D personality in women with breast cancer. The sample comprised 138 women with breast cancer, aged 23 to 79 years. Neuroticism and negative affectivity were associated with maladaptive forms of mental adjustment, specifically characterized by higher anxious preoccupation and feelings of helplessness/hopelessness. Emotional instability, sensitivity to threat, and a persistent tendency toward negative affect may hinder the adaptive cognitive appraisal of the disease and intensify perceived loss of control. Type D personality represented a vulnerability profile in which negative affectivity and social inhibition were linked to heightened emotional vulnerability, reduced social engagement, limited inclination for self-expression, diminished cooperativeness, and lower cognitive flexibility. Examining personality factors in women with breast cancer is critical for advancing the scientific understanding of individual differences in psychological adjustment and for informing clinical practice. Early identification of patients presenting with high levels of neuroticism, negative affectivity, and social inhibition can facilitate the design of targeted psycho-oncological interventions aimed at reducing anxiety and hopelessness, mobilizing social support, and fostering adaptive coping resources.
