Summary/Abstract: The aim of this study is to examine the Cognitive Behavioral Therapy process conducted under supervision with a client showing symptoms of health anxiety on the basis of case formulation. In the study, a single-case study design, one of the qualitative research designs, was adopted; the client’s cognitive, emotional, and behavioral changes during the therapy process were examined within their own clinical context. The data collection process was carried out through semi-structured clinical interviews, in-session observations, and the client’s verbal reports. During the eight-session therapy process, the client’s fear of death, anxiety about being alone, tendency to catastrophically interpret bodily symptoms, disruptions in sleep patterns, safety-seeking behaviors, and avoidance behaviors were identified as the main areas of focus. In the intervention process, psychoeducation, identification of automatic thoughts, establishment of the thoughtemotion-behavior relationship, cognitive restructuring, behavioral experiments, and homework assignments were used. At the end of the process, although the client’s anxiety symptoms did not completely disappear, it was observed that they began to evaluate bodily sensations in a more functional way, developed the ability to recognize automatic thoughts, and could more clearly identify the safety behaviors that maintained their avoidance behaviors. Supervision support contributed to deepening the case formulation, structuring the interventions according to the client’s needs, and maintaining the therapeutic process within ethical boundaries. The findings indicate that Cognitive Behavioral Therapy practices based on case formulation and supported by supervision may be clinically functional when working with symptoms of health anxiety.
