Summary/Abstract: This study introduces the Trojan Horse Model of Occupational Accident Analysis, aiming to overcome the limitations of traditional accident investigation methods. The model divides accident causes into two main categories: easily identifiable observable causes (unsafe acts and conditions) and unobservable causes deepseated systemic deficiencies (inadequate management, punitive safety culture) that are hidden within the organizational structure and pose a continuous threat. The Trojan Horse metaphor symbolizes how organizations unknowingly "take in" and normalize these unobservable risks. While bridging concepts from approaches like Heinrich and Reason, the model argues that observable failures are merely the manifestation of latent and systemic weaknesses. Its objective is to enable safety professionals to focus not only on immediate symptoms but also on the profound cultural and managerial issues at the root of accidents, thereby guiding the development of more effective, permanent, and proactive prevention strategies.
