Summary/Abstract: Submitted article presents a summary of the diploma thesis focused on the process of formation of multidisciplinary team, which becomes an instrument of care to services dealing with mental illness treatment in the frame of the transformation of psychiatric care from institutional to community services. In mental health community-based services as we know them today, multidisciplinatory cooperation should provide more complex and effective help for people with mental illness. The theoretical part of the thesis consists of informative overview, stating the basic terms in the field of mental disorders and community psychiatry, and the description of state-of-arts in caring about mentally ill. It also brings new information about planned reforms in the area of providing psychiatric services. The empirical part maps the process of putting the multidisciplinary team together in the newly established Centre of mental health, a new low-threshold pillar of transformation of psychiatric services. In addition, it shows views of all professionals from the team on the team development, mutual cooperation, and involving a peer consultant and it maps the barriers keeping multidisciplinary cooperation from optimal functioning as well.
