Summary/Abstract: Organic lesions in the vessels, localized mainly within arteries, are responsible for more than half of cases of erectile dysfunction. Balloon angioplasty of the arteries supplying blood to the penis, mainly internal pudendal artery has become one of methods used in the treatment of ED. This therapeutic method may be particularly useful in patients, who do not respond properly to conservative treatment with the use of phosphodiesterase inhibitors. The angiography remains the standard in evaluation of patients considered for angioplasty. Doppler ultrasound (with the intracavernosal injection of prostaglandin E1) and angio-CT are also used. Angioplasty of pudendal artery is similar in process, preparation, and contraindications to other endovascular procedures performed on peripheral arteries. The patient can return to normal activities within 24 hours after the procedure. Percutaneous angioplasty of arteries proved to be effective in restoring sexual function in 57% and even up to 100% of patients depending on the location. Ongoing ZEN study (Zotarolimus-Eluting Stent System for the Treatment of Erectile Dysfunction In Males with Sub-Optimal Response to PDE5 Inhibitors) has been launched to assess the effectiveness of drug-eluting stents in the treatment of erectile dysfunction in men with unsatisfactory response to phosphodiesterase inhibitors. Clinical trial results will be published later this year.
