Summary/Abstract: The demands for the safety operations of aeronautical complex systems at high hazards are always in place. Not considering the role of human factors and implicitly of the organization for the safety of the system during its progress, there always will be accidents. The challenge in reducing the unwanted events is actually represented by the social model of man-machine environment. Even if the system is designed for safety, the pressure for performance, high efficiency and objectives changing create vulnerabilities or major risks resulting in a poor level of safety.
