Summary/Abstract: Burnout in healthcare and helping professions has been predominantly framed as an individual deficit, a failure of personal resilience. This paper argues that such conceptualization obscures the systemic and neurobiological dimensions of occupational exhaustion. Drawing on Stephen Porges’ Polyvagal Theory, we examine how the state of the autonomic nervous system shapes individual and collective capacity for collaboration, empathy, and effective functioning. We further distinguish between burnout and moral injury, identifying the latter as a more accurate framework for understanding distress rooted in organizational betrayal, ethical conflict, and loss of professional autonomy. Integrating Transactional Analysis and transformational leadership theory, we explore how specific relational behaviors of leaders, ncluding double-bind messages, public humiliation, and implicit threats of replacement, function as chronic neuroceptive danger signals that dysregulate the nervous system of employees. We also examine how individuals with trauma histories may normalize toxic hierarchical dynamics, developing learned helplessness. The Balint group is reframed as a space for restoring empathic capacity through the exploration of the doctor-patient relationship. The paper concludes with a call for bottom-up, team-level changes that prioritize psychological safety and co-regulation as neurobiological conditions for professional vitality.
