Summary/Abstract: Acne is a chronic, inflammatory condition affecting the pilosebaceous unit of the skin. They are the most common diagnosis encountered daily in dermatology practice, and due to the complexity and diversity of clinical presentation requires individual approach and treatment of patients. The exact cause of acne has not yet been singled out, but it is known that it is a process involving several important factors. The ethiopatogenesis of acne is multifactorial. Key factors in the pathogenesis of acne are colonization with Propionibacterium acnes , keratinocyte hyperproliferation and follicular plugging, increased sebum production due to androgenic stimulation, and inflammation. They may also be associated with certain environmental factors such as some drugs, use of inadequate cosmetics (comedogenic) family history of acne genetics plays an important role in acne (increased risk of acne occurs in teenagers whose parents had acne), smoking, stress, pollution, nutrition (especially, foods with high glycemic index). Androgenic hormones play a major role in the pathogenesis of acne. The hyperproduction of sebum may be related to increased level of androgen hormones or hypersensitivity of the sebaceous gland to normal androgen levels or increased activity of 5α dihydrotestosterone (5α DHT) in the skin. Although in the majority of patients do not have underlying endocrine dysfunctions, it is important to be aware that acne may be a presenting symptom in persons with androgen mediated disease. Hermaphroditism is a rare disease where an individual has testicular and ovarian tissue and differs a few types. Patients diagnosed with hermaphroditism, depending on the indications, are placed on hormone therapy, in certain cases with testosterone.Here we will present a case of a patient diagnosed with hermaphroditism who received supplementary testosterone therapy in which nodulocystic acne acutely occurred after receiving a high dose of testosterone, as well as our approach to the treatment of them with oral retinoid therapy and the treatment of residual scars with combined laser and regenerative methods. This case indicates the direct impact of androgens or testosterone in etiopathogenesis of acne and how its normalization leads to their reduced occurrence. Modern methods shorten the healing time and intensity of residual scars.
