Summary/Abstract: The article discusses the content and form of health information in a normative scope, with particular emphasis put on information provided before high-risk interventions, using information forms. The patient’s right to information, defined by Polish legal regulations, and the manner of its implementation by representatives of medical professions have been presented. An important element of the considerations was the concept of consent to a medical intervention together with the assessment of the possibility of expressing this consent through a proxy or in a conditional health declaration. In the final part of the article, the medical forms were analyzed in the context of their compliance with key legal requirements of health information provided to the patient. In addition, a study was undertaken to examine whether the replacement of providing health information by sole usage of the general information form is acceptable in the light of the patient’s rights, in particular their right to decide (autonomy). The result of the research was that such practice is unacceptable.
