Possibilities for prevention of negative social consequences to patient upon discharge from hospital

Summary/Abstract: The activities of medical establishments is referred to taking care of disability persons and elderly people suffering chronic diseases or diagnosed definitely with continuous social consequences. A number of problems have been shown in a report of the governmental medical establishments of the country that negatively impact the subsystem of outpatient medical cares. A great number of the problems in the organization of medical sectors could be overcome through inclusion of social worker in the multidisciplinary team of the medical establishments.
