Summary/Abstract: Choice, efficiency and equity are important indicators in evaluating health care financing system. There is no single health care financing arrangement which is capable of achieving efficiency without undermining at the same time equity in financing and access to health care. These three indicators are highly interdependent and every attempt to improve one involves trade-offs within the system. Where the system will position itself it is a matter of political, social and economic circumstances of the country. Social Health Insurance (SHI) and Private Health Insurance (PHI) are two opposite ways of financing health care, where the former is known to be governed by the notion of solidarity and cross subsidisation between members of the scheme, while the latter based on willingness to pay, individualistic benefits and profit. Although SHI is facing increasing difficulties with its financial sustainability caused by reliance on a relatively narrow financing base (employment based coverage), recent reforms in some European countries makes it still a viable solution. At the same time, PHI might play an important role in reducing the total financial burden on Governments through different supplementary or complementary health insurance coverages.
